MEDICAL DECLARATION BY QR-CODE UNDER THE REGULATIONS OF THE MINISTRY OF HEALTH

* Instructions for registration of QR-Code information in public places:
 - For restaurants, accommodation (hotels, motels, inns) ... can perform "Registration of quarantine points" by:
 • Step 1: Access to https://tokhaiyte.vn to declare information (Name of translation control point / Phone number / Contact address required must be accurate).
• Step 2: The system provides the unit with a QR-Code.
• Step 3: The participants paste the QR-Code in the most visible place for people to scan.
Note: in case of printing to paste, it can be done directly on the authorized person's phone to scan.

* Instructions for use of medical declarations: People can take 2 forms to be able to "make a medical report".
 (Information about the history of the declaration will be kept in “My Statement”).
* Performing Medical Check-in / Check-out
 - When arriving at places that need confirmation of arrival and departure, also known as "Medical Check-in / Check-Out", people use the function "Scan QR" on the systems to declare.
- In the school where there is no smart phone, people can use the computer at the place where they need to declare access to the system (tokhaiyte.vn) to print QR-Code for confirmation or read the phone number to be confirmed.
•    Option 1: Access the link https://tokhaiyte.vn to declare online.
People can make "medical report" by using web browsers such as Firefox / Coc Coc / Chrome to access the link https://tokhaiyte.vn on computers or mobile devices (smart phones, tablet) with internet connection.    •    Method 2: Visit the Google Play app market (Android) / App Store (iOS) to find and download the application "Vietnam Health Declaration" "Bluezone" "Ncovi" to install the application on your mobile phone. Residents can make “medical reports” on their mobile phones by downloading applications to their personal computers.
•    Step 1:
(1) Access the address https://tokhaiyte.vn
 (2) Enter the correct mobile phone number to receive the OTP (Security Code) from the system.
(3) The system sends back the OTP code to the people through the message "IT-antiNCOV" on the phone.
(4) Enter OTP code to enter the system.
 
•    Step 2:
(1) Make accurate information updates: Full Name / Citizen Identification Number / Year of Birth / Sex ... (Or have a relative declaration)
Note: In case of remembering the Health Insurance Card Number, people can choose “Have a health insurance card” to enter the CARD NUMBER information and the system will automatically update personal information.
(2) Continue to update other information on the declaration to confirm sending information.
(3) Upon completion, the system will automatically return 01 QR-Code to each resident, with this code, “Medical Check-in / Check-Out” can be done at places where notification is required.    •    Step 1:
Visit the Google Play app market (Android) / App Store (IOS) to find and download the application with the name “Vietnam Health Declaration” “Bluzone” “Ncovi” to install the application on your mobile phone.
•    Step 2: Update accurate Mobile Number to receive Authentication Code (OTP) and access the system.
•    Step 3: Select the function "Medical declaration" to update necessary information with a sign (*) according to the form issued by the Ministry of Health and "Send information".
Note: With the software "Vietnam Health Declaration" that can remember the Health Insurance Card Number, people can select "Have a health insurance card" to enter the CARD NUMBER information and the system automatically updates the personal information.
•    Step 4: After submitting information, the system will automatically return a "QR-CODE" code to the people who can use "Medical Check-in / Check-out" at the required places.
